Welcome to Fridley Public Schools' Transition Program
The Fridley Transition Program is part of the Fridley School District. The program focus on assisting students 18-21 who have completed four years of high school as they progress from an educational setting toward living and working as independently as possible in their community. The program provides experiences that will enable young adults to live independently in the community.

PROGRAM GOALS:
- Focus on student needs
- Provide real life situations
- Encourage self awareness and independence
- Promote self awareness and independence
- Promote self advocacy and responsibility
- Build positive relationships between staff, students, families, employers and the community
- Explore opportunities for new experiences
- Collaborate with high schools to create a comfortable transition between programs
TO QUALIFY, A STUDENT...
- Must be 18-21 years old
- Must have completed 4 years of high school
- Must have an active Individual Education Plan (IEP)
(The Transition Team will decide if the student needs additional life-skills training that is necessary to live independently)
